This proposal responds to a documented gap in preventive-care access. A 2025 county assessment found up to 37% of adults skipped preventive care concentrated in rural, underserved communities (County Health Office, 2025). This gap maps to the fund's priorities of rural access and measurable reach (Community Fund guidelines, 2026).
