Statement of Need

This proposal responds to a documented gap in preventive-care access. A 2025 county assessment found up to 37% of adults skipped preventive care concentrated in rural, underserved communities (County Health Office, 2025). This gap maps to the fund's priorities of rural access and measurable reach (Community Fund guidelines, 2026). Comparable counties have narrowed similar gaps with a nurse-led mobile clinic model (Rural Care Access, 2024).
